We accept most major insurances, Medicare and Medicaid. We encourage all patients to confirm their insurance coverage prior to treatment. Please bring your insurance card and referral, if necessary, to your appointment. We also encourage you to arrive early to your first appointment to complete all necessary paperwork. Please bring a list of all current medications and any pertinent lab and x-ray results or medical documents you have.

As a courtesy to our patients, we will submit your insurance claims for you. It is important that you provide us with the name, address and telephone number of your insurance company and policy number. Since insurance coverage is a contract between you and your insurance company, it is your responsibility to remit payment for your balances not covered by your insurance company. Our fees reflect the usual and customary range for our region and indicate the level of care which is provided to you. If your insurance plan requires you make a co-payment or pay co-insurance, please do so at the time of your visit or you may be asked to reschedule your appointment. Medical record information will not be released without prior written consent. Some reports or copies of medical records require payment in advance. Please allow two weeks for this service.
