What to Bring to Your Appointment

• Your insurance card and co-pay or appropriate payment.  See “Financial Policies”.
• Current drivers license or identification card .
• Physician referral forms, if required by insurance or Medicaid.
• List of prescriptions and/or over-the-counter medication, including dose and frequency
• Information about your medical and surgical history .
• Recent x-rays or relevant records .
• New Patients should also bring the appropriate forms – confidential communication, new patient registration, patient information and medication forms completed or sent prior to your visit. The forms are available under the Patient Information and Forms button on this website.
